Course Development Forms
These forms are to be filled out in sequence (phase 1 then phase 2) during the process of online course development approval and planning.
These forms were approved for use in October 2014 and have been distributed to academic unit heads for distribution. Phase 1 is a preliminary approval process. Phase 2 is to filled out after phase 1 approvals. Phase 2 includes more in depth information on the nature of the course development, as well as course development payment procedures and the like. However, faculty may also download them themselves at this site.
